Full job description
Job Purpose:
To provide comprehensive case management services for clients and families enrolled in Lighthouse of Pinellas youth programs, including Early Intervention, Children’s Program, and Transition Program. This role ensures smooth experience from referral through service delivery by accepting referrals, completing intakes, and connecting eligible clients to the Division of Blind Services (DBS). The Case Manager provides community resources to parents and caregivers to help them better support their child, acts as a liaison with DBS counselors, and attends IEP meetings when needed. Additional responsibilities include supporting program staff with planning and organizing events and summer programming, assisting during staff absences, and generating program reports to track demographics, service utilization, and client outcomes.
Essential Functions:
- Complete new client intakes, coordinate service provision, provide information and referrals.
- Compile and maintain client and family communication, Apricot and AWARE reporting as needed.
- Maintain an updated spreadsheet with new client information to be printed and provided to instructors for ease of access in case of an emergency.
- Provide all medical, liability, photo release or any other pertinent information received (initially and annually) to be easily accessed by instructors for activities with clients.
- Monitor and document clients’ progress through their rehabilitation program through meetings with client, staffing, progress reports and collaboration with other staff members and the Division of Blind Services.
- Support the collaborative efforts of Youth Programs Event planning.
- Serve as the primary liaison and advocate between the Agency, the clients, their families and other agencies.
- Complete statistical information accordingly with DBS and Agency requirements.
- Schedule client transportation.
- Provide information and referrals to callers who inquire about the program and other community services.
Generals
- Provide community education/outreach through scheduled presentations (~4/yr).
- Attend required Agency meetings and training.
Qualifications:
Education:
- Bachelor’s Degree in Human Services, Social Work, or a related field
Experience:
- 2 years’ field experience working with children, preferably children with disabilities.
Demonstrated Knowledge and Skills:
- Excellent written and oral communication skills
- Excellent interpersonal skills
- Effective time management skills
- Ability to work as a team member
- Working knowledge of Microsoft Office applications
- Ability to use Internet for research
Physical Demands:
- Lift equipment used in performing job responsibilities
- Work extended and varied hours
Requirements:
- Pass background checks
Employee is responsible for timely transportation to and from all community-based instruction, training, education, and presentations.
- If privately owned motor vehicle is utilized for employment-related travel, the driver must possess a valid Florida driver’s license, good driving record, and proof of the following level of motor vehicle insurance: $100,000 / $300,000 bodily injury, $50,000 property damage, and work purposes endorsement.
